Athletic Participation Forms for 2018-2019
Students interested in participation in NSAA athletics and activities for the 2018-2019 school year need to complete the following requirements. 1. A school physical needs to be completed after May 1 for the upcoming school year. 2. The NSAA Student and Parent Consent form also needs to be completed. Please click on the links below to access each form. Students need to have these items completed prior to participating in any practices for the 2018-2019 school year. NSAA fall athletic practices begin on August 6, 2018. Please call the high school office at 402-269-2381 if you have any questions.
